The medial malleolus is easily palpated and observed as a large prominence medially. About 2 cm distal to the medial malleolus, the sustentaculum tali can be felt, especially if the foot is held in an everted position. The tibiocalcaneal portion of the deltoid ligament passes from the malleolus to the sustentaculum tali.
