Prophylactic Antibiotic TreatmentsControl of CAUTIs has become a major challengedue to the development and dissemination ofantibiotic resistance among the bacteria thatcause health care–associated infections (HAI).9,116-118 Prophylactic antibiotics have been shown topromote the development of resistant organisms.8Therefore, antibiotic stewardship initiativeshave been developed to reduce the unnecessaryuse of antibiotics to curb development ofresistant pathogens.119 Current CDC guidelines onmanagement of CAUTI do not recommend the useof routine prophylaxis with systemic antibiotics forprevention of CAUTI in patients requiring shortor long-term urinary catheterization, unless clinicalindications exist (e.g., in patients with bacteriuriaupon catheter removal post urologic surgery).108Additionally, there is no recommendation foruse of antimicrobial prophylaxis at the time ofcatheter removal.120 Moreover, the CDC does notrecommend the routine irrigation of the bladderwith antimicrobial agents.108 Therefore, bestpractice is to reduce unnecessary catheterizationand minimize dwell time.Antimicrobial-Coated Urinary
