This memorandum is to document our discussion of this date regarding your performance of duty. At the meeting, certain unsatisfactory aspects of your employment performance were pointed out to you. The problems discussed included :
1. Neglect care of child to cause my daughter to be suffering and injury to her health (Chapter 212 Section 27)
2. Habitual neglect of your duties
3. Dishonesty
4. Willful disobeys a lawful and reasonable order
This is not the first warning that you have received concerning your employment with the company. Future violations may lead to immediate dismissal without further notice or warning.
