Hello everyone,
In line with the continuation of our Health and Wellness Campaign, we have scheduled an Annual Physical Exam (APE) for all employees who missed the schedule last May. This will be on August 7th, from 12noon to 9pm at the 20th floor.
Our APE will include the following:
• Complete Physical Examination
• Complete Blood Count
• Urine Examination (for females, last day of menstruation should be at least 7 days prior to urine exam)
• Stool Examination (specimen must be submitted within 2 hours from time of collection)
• Random Drug Test (assignees will be drawn during the APE)
• ECG (for 35 years old and above, or if prescribed)
• Fasting Blood Sugar (for 35 years and above, or if prescribed)
• Pap smear (for females, 35 years and above, or if prescribed)
• Chest X-Ray (Mobile X-ray is located at the ground floor, near Elements Centris)
For all operations employees, your schedule is plotted by WFM on IEX while support staff will receive an email notification. Kindly also refer to the attached APE flow chart for your guidance.
Important Reminders:
• Bring your Avega Card/valid ID and stool sample on the APE. You may get your specimen containers from HR Office.
• For those who will undergo Fasting Blood Sugar, fasting for 8 to 10 hours is required (no food and water). Patient should be from rest, regardless if overnight or daytime.
• For female employees, kindly have your Physical Exam done first prior to X-ray procedure.
• If you are unable to attend the APE or failed to complete your exam, kindly get in touch with the nurse on duty. Employees who were not able to complete the onsite medical exams will be required to go to the nearest Avega Clinic for completion of APE.
• Regular employees who were hired from November 2014 onwards AND have undergone their Pre-Employment Medical Exam are exempted from taking the Annual Physical Exam.
