According to James,93 the talar head in a pronated foot can be palpated as a medial bulge; in a supinated foot the talar head bulges laterally. In the neutral position the talar head can be palpated equally on the medial and lateral aspects of the ankle. The neutral position is usually present when the longitudinal axis of the lower limb and the vertical axis of the calcaneus are parallel. This method for establishing the subtalar neutral position is useful in both the open-chain and closed-chain positions.
