Just distal, and slightly anterior, to the malleolus, a rather marked depression can be felt if the foot is relaxed. This is the lateral opening of the sinus tarsi. Traversing the lateral aspect of the sinus tarsi are the inferior bands of the extensor retinaculum and the cervical talocalcaneal ligament. If the palpating finger is moved around dorsally and slightly superiorly from the sinus tarsi, the lateral aspect of the neck of the talus can be felt, where the often-injured anterior talofibular ligament attaches.
